But it's 99% not a composer issue (never say never and all that. )

Composer doesn't do anything special with saving, only with reading "referenced files" such as images, transcluded files, etc. the operating system handles file writes in composer (MultiMarkdown Converter does a few other things but not composer)

If you haven't rebooted try that. Can also try disk utility and repair any issues or permissions problems.

Can try removing and reinstalling composer. Then restore purchases. That might reset things.

You've had a few OS issues (e.g. App Store and probably this one). Have you upgraded in place a few times? E.g. 10.9 to 10.10 to 10.11 etc? In my older machines I've learned that upgrading more than once or twice seems to increase the incidence of difficult to explain issues. When I do a complete reinstall of the OS everything seems to run better.

That's all I've got at the moment.
